Error using ADDT

Any tips and tricks that has to with the ADDT that doesn't fit into one of the other categories

Error using ADDT

Using ADDT in DW CS4 (only extension in DW CS 4). It used to work perfectly. But now when making list and form, the form keeps throwing this error:
Warning: Cannot modify header information - headers already sent by (output started at /usr/home/testworks.nl/htdocs/content/collmainform.php:1) in /usr/home/testworks.nl/htdocs/includes/common/KT_functions.inc.php on line 464
On line 2 is: <?php require_once('../Connections/cats.php'); ?>
Same is on the listpage and generated by ADDT.

Can anyone pse help solve this problem??

What do you have on line 1?
A blank space maybe?
